Privacy Policy

ADHD Herts respects your privacy and commits to protecting it through our compliance with the practices described in this policy. This is reviewed regularly and adheres to the guidelines outlined by the Information Commissioner’s Office (ICO) and the Health and Care Professions Council (HCPC).

This policy describes the type of personal information we hold, why we hold it, what we do with it and the controls in place to keep it safe.

Information routinely collected

  • Your/your child’s full name
  • Your telephone number
  • Your email address
  • Your/your child’s postal address
  • Your relationship with your child
  • Your/your child’s date of birth
  • Your/your child’s gender
  • Your child’s school
  • Name and contact details of other professionals involved in your/your child’s care.
  • Copies of relevant health correspondence which you have chosen to provide us with

How we obtain the information

  • When you register with us by completing our registration form or submitting an enquiry
  • Completing relevant forms and questionnaires relating to you/your child’s clinical care
  • Copies of any documents or letters which you bring to your appointment
  • Information from relevant professionals or school including returning forms and questionnaires relating to your care

We use several electronic questionnaires and a computerised task as part of the assessment.

Information sharing

We will only share your information on a need-to-know basis and will limit any information that we share to the minimum necessary. We may share information with other professionals in the following circumstances:

  • With your permission, we will write to your/your child’s GP following the assessment 
  • We can provide information directly to other agencies if you request. You may withdraw your consent at any time by informing Dr Holt or Dr Beak
  • We sometimes have a duty to share relevant information if there are concerns about safety and you will be made aware of this

Data storage

Your information is stored using Zanda (https://zandahealth.com/uk/security/) a secure GDPR-compliant software for healthcare practices. Any documents sent via email will be password-protected. All handwritten notes are anonymised and shredded after each session.

Clinical notes/reports are stored for six years post-treatment in line with guidelines provided by our professional indemnity insurance.

No data is ever sold or shared with third parties.

Your data protection rights

You have the right at any time to request access to, rectification or erasure of your personal data; to restrict or object to certain kinds of processing of your personal data.
